What Is Sagging Skin?
Sagging skin, also called skin laxity, is skin that has become less firm and no longer sits as tightly against the underlying facial or body structures.
One of the key changes happens in the dermis, the middle structural layer of the skin. The dermis contains an extracellular matrix, or ECM, made up largely of collagen along with elastin and other supporting components. This network gives the skin strength, elasticity and structural support.
Collagen accounts for approximately 70% of the skin’s dry mass, and research suggests that skin collagen may decline by approximately 1% per year throughout adult life. As collagen becomes reduced and fragmented with age, the skin gradually becomes thinner, less resilient and less able to maintain its original position. This can lead to:
- Softer facial contours
- Early jowls
- A less defined jawline
- Loose skin under the chin
- Neck laxity
- Drooping around the brows
- Fine lines and wrinkles
However, sagging does not occur in only one layer. Facial aging can also involve changes in fat compartments, ligaments, muscle-related support and bone. This is why effective facial rejuvenation often requires looking at the face layer by layer rather than simply treating what we see on the surface.
What Causes Sagging Skin?
The most common cause of sagging skin is aging, but sun exposure, weight changes, genetics and lifestyle can accelerate the process.
As we age, collagen and elastin gradually change and the dermis becomes thinner. At the same time, facial fat, ligaments and bone structure can change, reducing the support underneath the skin. Several factors can contribute to sagging skin:
Natural Aging
Collagen, elastin and other structural components of the skin change over time. Research has estimated that skin collagen decreases by approximately 1% per year during adulthood. Skin gradually becomes thinner, less elastic and less able to return to its original position.
Sun Exposure
Repeated ultraviolet exposure contributes to photoaging and damages collagen and elastin. Over time, this can contribute to wrinkles, uneven skin tone and loose skin.
Facial Volume Loss
Fat compartments in the cheeks, temples and other areas of the face can lose or redistribute volume with age. When structural support decreases, the overlying skin may appear looser.
Weight Loss
Significant or rapid weight loss can leave more skin than the underlying tissues can support, particularly around the face, neck, abdomen and arms.
Genetics
Some people naturally develop skin laxity earlier than others because of differences in skin thickness, facial structure and inherited aging patterns.
Smoking
Smoking contributes to premature skin aging and can damage collagen and elastin.
Pregnancy and Body Changes
Stretching of the skin during pregnancy or major weight fluctuations may lead to laxity, particularly around the abdomen.
What Does Sagging Skin Look Like?
Sagging skin usually looks like skin that has lost some of its firmness, definition or ability to “bounce back.”
- A less defined jawline
- Early jowls
- Lower cheeks appearing heavier
- Skin collecting around the nasolabial or marionette area
- A softer transition between the chin and neck
- Loose skin underneath the chin
- Neck skin becoming looser or crepey
- Drooping around the brows
- Facial contours looking less defined than before
How Can You Check for Sagging Skin By Yourself?
The Gentle Lift Check
Place one or two fingers near the outer cheek and gently move the tissue slightly upward toward the temple.
If the jawline, jowls or folds improve when the tissue is gently elevated, skin and soft-tissue laxity may be contributing to your concern.
You can also gently pinch a small amount of cheek or neck skin and release it. Skin that feels thinner, looser or slower to return may indicate reduced elasticity.
These tests are only general clues. Hydration, skin thickness, facial fat and natural anatomy can all affect what you see. A professional assessment is more useful because what appears to be “sagging skin” may actually be caused by volume loss, excess fat or several concerns occurring together.
Sagging Skin vs. Volume Loss: What Is the Difference?
Sagging skin and volume loss often happen at the same time, but they are not the same condition. Sagging skin is mainly a loss of firmness and support that allows tissue to sit lower. Volume loss means there is less underlying fullness supporting the face.
For example, a softer jawline or jowls may be related to laxity. Hollow temples or flattened cheeks are more suggestive of volume loss. In many patients, both occur together. The most natural-looking approach is a combination treatment plan that addresses both structure and skin quality.
| Sagging Skin | Volume Loss | |
| Main issue | Reduced firmness and tissue laxity | Loss of facial fullness and structural support |
| Common appearance | Jowls, loose neck, softer jawline, drooping tissue | Hollow cheeks, temples or under-eyes; flatter midface |
| Common areas | Jawline, lower face, neck, brows | Cheeks, temples, under-eyes, chin |
| Typical treatment approach | Ultrasound, radiofrequency, RF microneedling or threads | Dermal fillers or collagen-stimulating injectables |
A Layer-by-Layer Approach to Sagging Skin
Our Medical Director Dr. Cho explains that a facelift alone cannot replace every cosmetic treatment. Surgery mainly addresses excess sagging skin and tissue repositioning, while non-surgical treatments can target different layers of the skin to support collagen, elasticity, texture and overall skin quality.
The dermis, or middle layer of the skin, contains the extracellular matrix, which is rich in collagen and elastin. Treatments such as Thermage® FLX and Ultherapy PRIME® can target these supporting tissues to stimulate collagen remodeling and improve firmness over time.

Sagging Skin Treatment Options at Signature Cosmetic Clinic
Sagging skin can often be improved without surgery when laxity is mild to moderate. For sagging skin treatment in Toronto, the most important first step is determining whether the concern is caused primarily by skin laxity, volume loss, excess fat or a combination of these changes.
Signature Cosmetic Clinic offers multiple skin tightening technologies and injectable options so treatment can be selected according to your facial anatomy rather than relying on a single approach. Treatments may use ultrasound, radiofrequency, collagen stimulation, injectable volume restoration or thread-based repositioning.
1. Ultherapy PRIME®: Non-Surgical Ultrasound Lifting
Ultherapy PRIME is a non-invasive ultrasound treatment used to lift and tighten areas such as the brow, under the chin and neck.
What makes Ultherapy PRIME different is its ability to combine microfocused ultrasound energy with real-time visualization. The provider can visualize tissue and select treatment depths of 1.5 mm, 3.0 mm or 4.5 mm, depending on the tissue being targeted. This means treatment can reach from collagen-rich dermal tissue to deeper fibromuscular layers, including the SMAS/platysma level.
Ultherapy PRIME may be considered for patients with mild to moderate facial or neck laxity who want lifting without surgery or injectable treatment.
2. Thermage® FLX: Radiofrequency Skin Tightening
Thermage FLX uses monopolar radiofrequency energy to heat deeper layers of tissue and support collagen remodeling. It is commonly used for the face, jawline, around the eyes, neck and selected areas of the body.
Thermage is particularly useful when the goal is overall tightening, smoothing and improved facial contour. Some tightening may be noticeable earlier, while collagen remodeling continues gradually over the following months.
3. Sofwave™: Mid-Dermal Skin Tightening
Sofwave is a non-invasive ultrasound treatment that delivers energy into the mid-dermis to stimulate a collagen-remodeling response. It can be used to improve skin laxity and is cleared for lifting areas including the eyebrow, submental area and neck.
Sofwave may be an option for patients who are beginning to notice mild laxity and want a non-invasive treatment with minimal interruption to their normal routine.
4. XERF™: Multi-Depth Radiofrequency Tightening
XERF is a non-invasive monopolar radiofrequency treatment that uses two RF frequencies, 6.78 MHz and 2 MHz, to deliver thermal energy at different tissue depths.
Its shallow, middle and deep treatment capabilities allow the treatment to be adjusted according to the patient’s anatomy and areas of laxity. XERF can be considered for facial and neck tightening, particularly when the goal is to address skin firmness at multiple depths.
5. RF Microneedling
Radiofrequency microneedling combines microneedles with controlled RF energy delivered into the dermis. The treatment creates controlled micro-injuries and thermal stimulation that initiate skin remodeling and new collagen formation.
RF microneedling may be particularly helpful when sagging skin occurs together with concerns such as enlarged pores, acne scars, fine lines or uneven skin texture.
6. Sculptra®
Sculptra is an injectable poly-L-lactic acid treatment that works differently from a traditional hyaluronic acid filler. Rather than simply creating immediate volume, it is used to gradually improve facial structure and stimulate collagen production over time.
Sculptra may be considered when facial aging includes both declining collagen support and volume changes, particularly through the cheeks and other areas of the face.
7. Dermal Fillers
Dermal fillers can help when what appears to be sagging is partly related to loss of underlying facial support.
Strategically restoring volume in areas such as the cheeks, chin or jawline can improve facial proportions and provide additional support to surrounding tissues. The goal should not simply be to “fill” loose skin. Professional assessment is important because excessive filler can make the face look heavier rather than more lifted.
8. Thread Lift
A thread lift uses absorbable sutures placed beneath the skin to reposition selected facial tissues. Unlike energy-based treatments, which rely mainly on gradual collagen remodeling, threads can provide a more direct mechanical repositioning effect.
Thread lifting may be considered for selected patients with mild to moderate facial laxity who want more visible repositioning but are not ready for surgical lifting. At Signature Cosmetic Clinic, thread lift procedures are performed exclusively by our Medical Director Dr. Cho.
Which Treatment Is Best for Sagging Skin?
There is no single best treatment for every case of sagging skin. The most appropriate treatment depends on the location and severity of laxity, skin thickness, volume loss, facial anatomy, age and desired downtime. Many patients do not fit into only one category. Combination treatments can be used when both skin laxity and facial volume changes are present.
| Treatment | Often Considered For | Technology / Approach | Treatment Type |
| Ultherapy PRIME® | Brow, jawline, under chin, neck; deeper lifting | Microfocused ultrasound with real-time visualization | Non-invasive |
| Thermage® FLX | Overall facial, eye and body tightening | Monopolar radiofrequency | Non-invasive |
| Sofwave™ | Mild facial, brow and neck laxity | Ultrasound delivered into the mid-dermis | Non-invasive |
| XERF™ | Multi-depth facial and neck tightening | Dual-frequency monopolar RF | Non-invasive |
| RF Microneedling | Laxity plus pores, texture, wrinkles or acne scars | Microneedles + RF | Minimally invasive |
| Sculptra® | Collagen loss and facial volume changes | Injectable collagen biostimulator | Injectable |
| Dermal Fillers | Structural or facial volume loss | Injectable volume restoration | Injectable |
| Thread Lift | Selected areas requiring tissue repositioning | Absorbable lifting sutures | Minimally invasive |
When Should You Consider Treatment for Sagging Skin?
You can consider treatment when you begin noticing changes in facial firmness or contour that bother you. You do not need to wait until sagging becomes severe. Common reasons patients seek treatment include:
- Your jawline looks less defined than before.
- Early jowls have started to appear.
- Your cheeks appear to sit lower.
- Skin underneath your chin feels loose.
- Your neck looks less firm.
- Your brows have begun to droop.
- Your skin no longer feels as firm when you touch it.
- You have experienced significant weight loss.
- Skincare is no longer giving you the improvement you want.
- You want to address early laxity before considering surgical treatment.
- Non-surgical skin tightening generally works best for mild to moderate laxity.
How Can You Prevent or Slow Sagging Skin?
Use Sunscreen Daily
UV exposure contributes to the breakdown of collagen and elastin and is one of the major preventable causes of premature skin aging. Use a broad-spectrum SPF 30 or higher every day and avoid tanning.
Avoid Smoking
Smoking accelerates visible skin aging and contributes to collagen and elastin damage.
Maintain a Stable Weight
Repeated large fluctuations in weight can stretch the skin and contribute to laxity.
Follow an Evidence-Based Skincare Routine
Retinoids, antioxidants, moisturizers and sunscreen can help improve several signs of photoaging and maintain skin quality.
Topical products can improve texture, hydration and fine lines, but creams cannot physically reposition significant loose tissue.
Eat a Balanced Diet
Adequate protein, vitamins, minerals and overall nutrition support normal skin health and collagen production.
Consider Preventive Treatments When Appropriate
Some patients begin collagen-stimulating or energy-based treatments when laxity is still relatively mild rather than waiting for more advanced sagging.
Treatment frequency should be personalized. More energy and more treatments are not automatically better.
Frequently Asked Questions About Sagging Skin
Can sagging skin be tightened without surgery?
Yes. Mild to moderate sagging skin can often be improved with non-surgical treatments such as Ultherapy PRIME, Thermage FLX, Sofwave and XERF. RF microneedling, Sculptra, fillers or thread lifting may also be recommended depending on whether the concern involves skin laxity, collagen loss or loss of facial volume.
Non-surgical treatments cannot reproduce the amount of tissue removal or repositioning possible with a surgical facelift, particularly when laxity is severe.
Are there non-invasive options for skin tightening on the face?
Ultherapy PRIME, Thermage FLX, Sofwave and XERF are examples of non-invasive facial skin tightening treatments. They use ultrasound or radiofrequency energy to stimulate collagen remodeling without surgical incisions.
The best technology depends on the depth and location of the laxity as well as your anatomy and treatment goals.
What causes sagging skin?
Sagging skin is most commonly caused by age-related changes in collagen, elastin and supporting facial tissues. Sun exposure, smoking, genetics, facial volume loss and significant weight changes can also contribute.
Facial aging involves more than the skin itself, which is why sagging and volume loss frequently occur together.
What is the best treatment for sagging skin?
There is no single treatment that is best for everyone.
Ultherapy PRIME may be selected for deeper ultrasound lifting, while Thermage FLX, XERF or Sofwave may be considered for different levels of non-invasive tightening. RF microneedling can help when texture is also a concern. Sculptra and fillers are more relevant when collagen or volume loss is contributing.
A facial assessment is the best way to choose between them.
How long does it take to see results from skin tightening treatments?
Most collagen-stimulating skin tightening treatments produce gradual results.
Some patients notice early changes after treatment, but improvement commonly continues over several weeks or months as collagen remodeling occurs. Timing varies depending on the technology, treatment area, age, skin condition and individual biological response.
Injectable fillers may provide more immediate structural changes, while Sculptra develops more gradually.
Do firming creams actually tighten sagging skin?
Firming creams can improve hydration, texture and the appearance of fine lines, but they cannot create the same tissue tightening or lifting effect as an in-office energy-based treatment or surgery.
A good skincare routine is still important because healthier, well-protected skin can complement and help maintain professional treatment results.
Does exercise help tighten loose skin?
Exercise can improve muscle tone and body composition, but it does not directly remove or tighten significant excess skin.
Building muscle may improve the appearance of some body areas by providing more underlying shape. Facial sagging, however, usually involves changes in skin, fat and structural support that cannot be corrected through exercise alone.
How can I tell if I have loose skin or excess fat?
Loose skin tends to look thin, wrinkled, folded or lax, while excess fat usually creates greater fullness or thickness underneath the skin. Under the chin, for example, a less defined jawline can result from loose skin, submental fat, facial anatomy or a combination of all three.
A professional consultation is the most reliable way to determine which factor is contributing most.
How can I tell the difference between sagging skin and volume loss?
Sagging skin causes tissue to look lower or looser, while volume loss creates hollowness, flattening or reduced facial support.
Jowls and a loose neck are often associated with laxity. Hollow temples and flattened cheeks are more commonly associated with volume loss.
However, both changes frequently occur together and may require a combination treatment approach.
At what age does facial skin start to sag?
There is no specific age at which sagging begins. Genetics, sun exposure, skin thickness, facial structure, lifestyle and weight changes all influence when laxity becomes noticeable. Some people notice early changes in their 30s, while others develop them considerably later.
Treatment should therefore be based on the condition of the skin rather than age alone.
Does exercise help tighten loose skin?
The cost of skin tightening in Toronto depends on the technology, treatment area and amount of treatment required.
For example, treating a small facial area is different from treating the full face and neck. Ultherapy PRIME, Thermage FLX, Sofwave, XERF and RF microneedling also use different treatment protocols.
A consultation allows us to recommend an appropriate treatment first and provide accurate pricing based on your individual plan.
